Age Navigation is a Care Management service developed by RN's who are Certified Care Managers and have personal and professional experience managing the medical and insurance systems. We have no financial interest in communities or agencies that provide direct services.

 Product and Service Description:

ONGOING CARE MANAGEMENT AND SUPPORT : Each family and situation is unique, therefore we work with you to provide the service you need. We provide: guidance for dealing with a particular situation, coaching and support .

ASSESSMENT and CARE PLAN - Our RN/Care Manager conducts an in-depth interview with the older adult and family members, taking a realistic look at the current living situation and support needs. We then give options and recommendations and help the family address the issues.

CRISIS Management - Call us us to help you make the best decisions when confronted with an unanticipated illness or injury. We help you navigate the medical and insurance world by asking the right questions and translating the answers for you.

BILL PAYING Service: We offer a cost-effective way for clients to get assistance with organizing, bill paying, balancing checkbooks, and reviewing statements. The process is monitored by a CPA.
